you are absolutely right that the combs shouldn't be smallish and pale. In
pullets it may take a while until their combs grow, but in general a small,
pale comb does indicate a problem, parasites or coccidiosis are common
problems and the symptoms may not always be very obvious.
You mentioned worming them with ivermectin in the water, unless you use a
water soluable form of ivermectin like the sheep drench, the worming you do
may not be effective, the 1 % injectable that is often recommended on
chicken lists does not mix with water, and on most other bird lists (non
poultry), they only recommend the water soluable forms of ivermectin and
moxidectin. So parasites could be the issue as your worming may not have
done what you expected it to do.
Coccidiosis comes in various forms and not all have obvious symptoms, so
you may want to consider treating them for coccidiosis to rule out that
this is the cause of the problem, or if you can, have fecals done on them
to see if the culprit can be identified.
